Using data from the Ministry of Health’s 2023 Indonesian Health Survey (SKI), researchers analyzed data from 89,866 adolescents aged 15 to 24 across Indonesia. Respondents’ depression status was measured using the Mini International Neuropsychiatric Interview (MINI), a standardized diagnostic tool based on international mental health criteria. Rather than analyzing gender, age, education, place of residence, and economic status separately, as most previous studies have done, the research team used cluster analysis to classify adolescents based on combinations of these characteristics simultaneously.

As a result, the researchers identified five groups of adolescents with distinct social profiles. The first group consisted of young male adolescents in rural areas with high poverty rates, which served as the benchmark group in the analysis. The second group consisted of older female adolescents in rural areas who remained poor despite having completed high school. The third group consisted of young female adolescents in urban areas from affluent families. Meanwhile, the fourth and fifth groups were dominated by male adolescents from affluent urban families, with the fifth group being the most advantaged both economically and educationally.

Compared with the first group, adolescents in the second group had a 1.58 times higher risk of depression. But the most striking finding came from the third group: young female adolescents from affluent urban families had a 2.6 times higher risk of depression than the most disadvantaged group. In contrast, the two groups dominated by males from affluent families showed no statistically significant association with depression risk.

The researchers explained that this finding shows depression risk among adolescents does not correlate directly with poverty alone, but is instead shaped by the intersection of gender, age, and the social context in which adolescents live. Among poor female adolescents in rural areas, the pressure stems from a double burden of economic constraints and sociocultural demands, such as upholding the family’s reputation and fulfilling household responsibilities, which in some regions even leads to early marriage after completing compulsory schooling.

For young, affluent female adolescents in urban areas, researchers suggested that the pressure instead stems from a crucial period of social transition, namely the shift from elementary school to the more complex social environment of junior high school. Notably, this group had the healthiest lifestyle among all five, with nearly all reporting no smoking or alcohol consumption whatsoever. This suggests that the elevated depression risk in this group is unlikely to be driven by risky lifestyle behavior, and more likely rooted in deeper social pressures and gender expectations.

Regarding the lower depression rates found in male-dominated groups, the researchers cautioned against interpreting this as evidence of greater mental resilience. Prevailing norms of masculinity often make adolescent boys reluctant to express emotional vulnerability, meaning that existing symptoms of depression may go undetected or unreported in surveys of this kind.

Based on these findings, the research team recommended that adolescent mental health programs not be designed uniformly, but instead tailored to the specific context of each group. For poor female adolescents in rural areas, they emphasized the importance of economic and women’s empowerment programs in rural communities. For young, affluent female adolescents in urban areas, the researchers suggested integrating mental health services directly into the school curriculum to help them navigate gender-norm pressures and social transitions from an early stage.

The research team also acknowledged several limitations of the study. Because of its cross-sectional design, the findings can only indicate association, not causation. In addition, the data, collected through respondents’ self-reports, may be subject to bias, particularly for sensitive questions such as smoking habits and alcohol consumption. As such, further research using a qualitative approach is considered necessary to gain a deeper understanding of the lived experiences of adolescents within each group.

Author: Prof. Ferry Efendi, S.Kep., Ns., M.Sc., Ph.D

Source: Murti FAK, Nursalam N, Kustanti CY, McKenna L, Sutrisno S, Pradipta RO, Saleh A, Efendi F. “Clustering socio-demographic of Indonesian adolescents and their associations with depression: a cross-sectional study.” BMJ Open 2026;16:e109727. doi:10.1136/bmjopen-2025-109727.

It is accessible at BMJ Open.

A nationwide study published in the journal BMJ Open has revealed a surprising finding that challenges a long-held assumption: female adolescents from affluent urban families actually face a higher risk of depression than poor female adolescents in rural areas.
